In 2012 the University Hospital of Salerno (UNISA) has started a Program for the proactive tackling of Cerebral and Vascular events in high risk populations. Within this program, a network has been established between the Hospital, general practitioners, local health authority of Salerno Department of territorial care and private home care provider. This network is supported by ICT in order to share clinical parameters of patients. This network has participated in different programs, including EC funded pilots such as Beyond Silos, regional programs of innovation such as ADD Protection, CUREDOM and the Italian Ministry of Health pilot EASYDOM. These pilots all involve the use of ICT for integrated care of chronic patients with multimorbidity and cardiovascular disease.
The University of Salerno is directly linked with So.Re.Sa, a member of the buyer group, and is responsible for the management of the Campania pilot site.
